New Patient Information


Welcome to Allergy & Asthma Care and Prevention Center. We are so pleased that you have chosen our office to help you with your allergy and asthma health needs!

Prior to your first visit, take some time to review the information below and complete the New Patient forms. Having this information prior to your visit will allow us to prepare more effectively for your time in our office.

1. Our new patient forms will be emailed to you prior to your first visit. Please complete forms online and submit back to us before your appointment. If for any reason you are unable to fill the forms out online, let our front office staff know and alternative arrangements can be made.

2. If your insurance company requires a REFERRAL to see a specialist, it is your responsibility to make sure that we have received the REFERRAL prior to your scheduled appointment.

3. If you have already had testing or laboratory work completed, be sure to have all of the results sent to us prior to your appointment, or bring the results with you to the appointment.

4. In most instances, we prefer to complete allergy testing at the time of the initial visit, therefore we ask that you avoid taking certain medications that interfere with this type of testing prior to your appointment. For the full list of medications please click here.

ON THE DAY OF YOUR APPOINTMENT, please plan to arrive 20 minutes prior to your appointment time so that you will be ready to meet with the doctor at your scheduled appointment time. During your visit, the doctor will listen to you and your concerns and recommend the appropriate testing options and treatment course for you. As a new patient, plan for your visit to take up to 2-3 hours although it may be less. The length of your visit will depend on which testing is appropriate for you. In some instances, additional testing during additional office visits will be necessary to best address your health concerns and symptoms.
